FCC Input File Requirements

The FCC Form 477 electronic filing system is now available for filing at the following address on the FCC’s Website: http://www.fcc.gov/formpage.html. Broadband Internet service providers and interconnected VoIP service providers who are required to file FCC Form 477 can also find instructions, systems documentation, FAQs, and other relevant information at www.fcc.gov/form477.

The input file to process must include the following data for your Broadband subscribers:

1. Unique ID
2. Address (2 line address or address and city, state & ZIPCode in separate fields
3. Technology code (see Technology Code list)
4. Upload speed (see Upload Speed Code list)
5. Download speed (see Download Speed Code list)
6. Residential / Business indicator

(If your S&E codes have the technology type, upload and download speed, BITI will translate
these codes into the appropriate FCC reporting Codes as part of the service.)
